UNIT 3 HEALTH BENEFIT FUND (HBF)
Effective September 1, 2024, funds are based on academic year (no longer based on calendar year) i.e., Members are eligible to apply once per academic year.
Important: Those who are eligible to apply for Unit 3 HBF will receive an email from the Unit 3 VP with the HBF claim form attached once the HBF becomes available.
- Unit 3 HBF is available each academic year from October 1 to August 15, or until funds are fully depleted.
ELIGIBILITY: Unit 3 Lab Monitors, Graduate Assistants, and Teaching Assistants having at least one course appointment in the term (with Unit 3 union dues being deducted from your biweekly pay), are invited to apply for Health Benefit Funds (HBF) of $700.
- Funds are available on a first-come-first-served basis; submit your claims early
- Should funds be depleted prior to the deadline the claim form will no longer be accepted
- Maximum of one claim per academic year; incomplete claims will be denied
- Members must use their @torontomu.ca email address when applying
- All Payments are issued by cheque and mailed directly to members by a third party benefit provider.
Please note: All Unit 3 HBF claim forms are accepted through email submission only to a third party benefit provider i.e. do not send the information to CUPE 3904. All Unit 3 HBF claim forms must be correctly and fully completed with all necessary information and documentation attached in the email including pictures, scans or PDF versions of original receipts or they will not be considered. Please make sure that you email the completed claim form and your electronic versions of original receipts to the third party benefit provider listed on the claim form.
